Do you have the sheet music on-hand?

Performers are required to provide their own sheet music for ad-hoc concerts. Indicating yes means you have the sheet music in your possession.

Faculty Member

Name of the faculty member that will hear the piece before the performance, if chosen to be programmed. Do not enter a name without first getting confirmation from faculty.

Conflicts with Performance Dates

If anyone in the ensemble has a conflict with any date, please indicate that here. We will do our best to accommodate conflicts. If no conflict, please write N/A.
